Dr. Malpani express views on patients complaints: Your gynecologist may be a threat to your fertility

Some of the Dr. Malpani complaints that circulate on the internet are received from gynecologists. They even go as far as to refer to Dr. Malpani cheater. Often they are insufficiently qualified to make such statements, as gynecologists are not always well equipped to treat complicated infertility problems. A large number of them will do an adequate job, but they can, at times, turn out to be an impediment in a couple’s efforts  to conceive.
For women, in most cases, gynecologists are the primary care physicians. In the event a couple faces challenges in the case of fertility, most often it is the woman who seeks medical advice, her gynecologist being the first doctor she consults with.
But gynecologists often do not have any specific experience or training to deal with infertile women. Oftentimes they are unable to formulate an effective treatment protocol, resulting in the fact that they often end up providing treatment on an ad hoc basis, causing the patient to waste precious time and money.
Additionally, most gynecologists are not really capable to deal with male infertility challenges. These men are often referred to andrologists who offer disjointed treatment and care, resulting in the couple receiving compromised treatment.
Additionally, a large number of gynecologists lack the expertise to take care of older women with infertility problems. Their main area of focus is that of delivering babies, and therefore they often do not take in consideration the damaging effect the passage of time can have on a woman’s ovarian reserve. As a result, gynecologists often provide ineffective treatment options such as clomid for ovulation induction, which results in these older women losing valuable time.
Since gynecologists prefer to retain control over their patients, they are often rather disinclined to refer patients to infertility specialists for IVF treatment. Many of them will recommend the removal of small endometriotic cysts or fibroids. These surgical procedures do however not improve the chances of the patient to conceive. In fact the chances of these women falling pregnant are  adversely affected by these types of surgery, as they result in the formation of scar tissue and adhesions which reduces ovarian reserve.
Some older gynecologists still make use of outdated or obsolete procedures such as hydrotubation for the re-opening of blocked tubes. All these treatments and surgeries can cause the patient to lose confidence in doctors because they do not address her infertility problem effectively . These women often give up infertility treatment all together, since they have lost a lot of precious time and money.
Gynecologists often also lack a holistic perspective and expertise when dealing with infertile couples. Their studies usually do not include information about  handling infertile men with low sperm counts.

Eventually, the couple either gives up altogether, or they will seek treatment from an IVF specialist. The loss of precious time, however, is irretrievable, and often results in older women missing their shotat falling pregnant.
